Incident Form

 

For ODGSL's protection and information, the "Accident/Incident" form was created to allow the capture of factual information. If during a game anyone is injured, please take time to report any such information to an ODGSL Board member for inclusion in our records.

Every coach received a copy of this form at the beginning of the season, these web forms are to allow for an additional avenue to return this information.

If you cannot access these forms, please contact the Secretary to request a copy of the document.

Upon completion of this information please contact the ODGSL President, Vice President, or League Commissioner (see Board of Directors) as soon as possible to coordinate the return of the form and to answer any additional questions that may arise related to the incident.
